Swimming Lessons Summer Suspension OPT OUT
The completion of this suspension OPT OUT form indicates that you wish to continue your regular Swimming Lesson direct debit payments for the period indicated.
An active membership over this time will allow you access to the pools at Carlton Baths, North Melbourne Recreation Centre and Melbourne City baths for no additional entry fee. So if you will be around over the summer, it’s a great opportunity for a visit to the local pools, to practice aquatic skills and have some fun in the water at the same time!
Please complete this form by Thursday 22nd December 2022.
If you do not complete a request to opt-out of the automatic suspension your final direct debit payment for 2022 has already taken place.
*Please note: if you attend your regular lesson on Thursdays or Saturdays, you will receive a one class payment fee on 15/12/2022.
